Technical Evaluation Criteria:
Section 1: Mandatory Criteria:
(A) Bidder must pass all of the mandatory criteria above to be technically acceptable.
(B) Failure of bidder to meet any of the mandatory criteria above will result in technical failure and no further evaluation.
1. 1. Bidders must have a minimum of five 5 years experience with technical knowledge in all aspects with the work, associated equipment and has experience within the Oil, Gas, heavy Industry. Bidders shall provide list of past and current service contracts as legitimate proof documents.
2. 2. Bidder shall confirm by letter that their Company complies to all Companys requirements as per the Scope of Work without deviations or stamp,sign each page in the tender package.
3. 3. Bidders employees should have a minimum Three 3 years experience in chemical treatment services. Bidder must submit evidence CVs and total years of personnel experience as specified in Annex B Contractor Personnel.
Section 2: Scoring Criteria:
1. 1. Submission of QMS questionnaire
2. 2. Bidder ISO 9001 certified (certificate provided)
3. 3. Bidder provided CV of the Quality Assurance Officer
4. 4. Bidder provided QMS copy as per ISO 9001
5. 5. Meets applicable HSE requirements such as HSE Plan-Contractors to submit a copy of their Company's HSE Plan including Skill / HSE Training Matrix, HSE Policy signed by contractor management
6. 6. HSE Questionnaire completed with acceptable responses: - Focus on IIR & Severity Rate information for the last 3 years and adequacy of bidder responses to the other questionnaire information requests. Evidences/ justifications are required for each question
7. 7. HSE Representative- Bidder Shall have in their full-time employment as part of their overhead structure, a NEBOSH or equivalent qualified Safety Officer with five (5) experience
8. 8. HSE Accreditations- List accreditations OHSAS 18001, ISO 45001.
